UnitedHealth lifted its 2026 earnings outlook for the third time this year after third-quarter profit jumped 75 percent even as revenue slipped, the clearest sign yet that the insurer's strategy of shedding unprofitable Medicare Advantage members is translating into margin expansion.

The numbers

Net income reached $4.1 billion in the quarter, up from $2.3 billion a year earlier, on revenue of $112.7 billion that edged down 0.4 percent. The medical loss ratio fell to 88.4 percent from 89.9 percent, meaning the company retained a larger slice of premium dollars. Optum Health posted $735 million of operating income, roughly eighteen times the year-ago figure, while UnitedHealthcare's operating profit climbed to $2.8 billion, a $1 billion improvement.

The Medicare Advantage trade-off

The insurance division expects to finish 2026 at the upper bound of its 2 percent to 4 percent margin target, president Bobby Hunter said on the earnings call. That outcome follows a deliberate contraction: UnitedHealthcare anticipates losing more than one million Medicare Advantage enrollees this year after raising premiums, cutting benefits and exiting unprofitable plans. For 2027, the insurer has eliminated hundreds more plans, shifted toward narrower-network HMOs and expanded special-needs plans for chronically ill and dual-eligible seniors, products that typically carry higher per-member margins.

Star ratings hangover

The quality-score backdrop is less favorable. The share of UnitedHealthcare members in plans rated four stars or higher drops to 68 percent for 2027 from 81 percent this year. Since those ratings determine bonus payments and competitive positioning, the decline is projected to pressure 2028 Medicare Advantage revenue.

Commercial pressure persists

Outside the government programs, the No Surprises Act arbitration process continues to weigh on the commercial business, executives said. Morgan Stanley analysts described the quarter as solid and said it reinforced confidence in the turnaround trajectory. Hunter characterized 2027 growth potential as substantial, though the company declined to provide membership assumptions at this stage.
